Special Conditions:

• Car accidents and Dental Treatments are excluded.
• The insured should be in full compliance with the local and state authorities, regulations and laws.
• Facilities with 100 children or more are subject to a different rating and underwriting manual.
• Important: the policy is in excess of any other insurance.

• Facility Equipment – for each 10,000 value – annual premium of 109 per year. Coverage includes fire and allied perils, nature’s hazards, storms, hail, earthquake, burglary, malicious damage and terrorism.
• Facility Building – for each 10,000 value – annual premium of 42 per year. Coverage includes fire and allied perils, nature’s hazards, storms, hail, earthquake, malicious damage and terrorism.
• Employers Liability – annual premium of 125 per employee.
